Please note we will only accept under 15s registrations if the parent/guardian is a registered patient with us.
For Students
You can register with us via the Campus Doctor site. If you choose this option, please complete the online registration forms here and send your photographic ID to the practice email within 7 days. Failure to provide the identification in this time scale means you registration will be invalid, and you will complete the forms again.
Please only fill out one registration form.
If you are a patient who is not "ordinarily resident" in the UK, there is an information leaflet available, as you should be aware that not every person is entitled to all NHS services free of charge in England.
National Data Opt Out
Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.
You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13. There is a link under resources or a page on the website called National Data Opt Out.
