How to...

Register with the Practice

If you would like to register with Newcastle Medical Centre, then it couldn't be simpler.

If you are a student you can register with us via the Campus Doctor site.  If you choose this option please complete the online registration forms on the link opposite and please remember to bring your identification to the Practice within 7 days.  Failure to provide the identification in this time scale means you registration will be invalid and you will complete the forms again.


First, check that you reside within the Practice Catchment Area if you are not then you can register however you will need to complete an out of area decleration form as some services will not be available.  The form is available for download

Then, download the following documents, (registration form, health check form form and T&C’s of registration), complete them and bring them along to the practice with two forms of ID. If you are from an EEC country you must also bring your EHIC card.  There is also a leaflet attached for  patients who are not "ordinarily resident" in the UK as you should be aware that not every person is entitled to all their NHS care free of charge in England.

Acceptable forms of ID are:

  • Passport

  • Visa

  • Student Card

  • Photographic Driving License

  • Proof of Address

Once registered we will give you an information pack, that will include details of summary care records.
